MyCare Insurance Program Application Form for
Individuals, Couples or Families


Thank you for your interest in joining the MyCare family!

Before you apply, it is important that you understand the pre-existing condition clause on the MyCare Insurance Program. Please read through our pre-existing condition terms:


A Pre-Existing Condition* means:

  1. Congenital and chronic medical conditions, and bariatric surgery
  2. Medical conditions for which the insured person received a diagnosis, medical care, services, medication, diagnostic tests or consultation prior to the inception date of coverage
  3. Medical conditions which produced symptoms prior to the inception date of the policy, whether a diagnosis was produced or not, by one learned in medicine
  4. Related medical conditions, which in the 24 months prior to the inception date of the policy, exists, is suspected to exist or has been diagnosed, are not covered by the policy. An example would be a heart condition that occurs as a result of diabetes


Due to the pre-existing condition clause, MyCare is not subject to any medical underwriting – therefore you will not be asked any medical questions and a medical exam will not required.

Our application process typically takes a few business days. Your coverage becomes effective on the first of the month following the month of applying with us.

Once approved and processed, you will receive a welcome package in the mail. The welcome package typically arrives within three weeks of your application being processed.
